A small but significant number of patients presenting to musculoskeletal examination suffering from a serious medical condition or disease. To filter these patients quickly the concept of red flags has been developed to increase the reliability of medical history and physical examination, thus reducing the likelihood that something was lost. The large increase in costs and disability that has accompanied the increase in the absence of low back pain at work has highlighted the development of tools for identity and counteract the negative influences in recovery from low back pain.
Psychosocial risks of long term disability and inability to work after an episode of back pain are those that identify yellow flags and its principles may be applied to other pain syndromes. Transfer of immediate medical consultation and treatment is aimed at identifying the red flags, to undertake a further assessment and treatment intervention is necessary in order to identify the yellow flags. The effort and time invested in identifying risk factors caution is wasted if there is no plan to accompany the cognitive and behavioral interventions.
Low back problems have important consequences that are the pain itself, the loss of functional activity and the reduction or loss of ability work. When you have acute back pain, the probability of becoming chronic is small, but significant, and our ability to intervene to change the course of episode may be limited. Analgesics have little effect on whether we are to develop chronic pain, but the initial acute pain control, and any can be a useful thing to do. Excessive pain lasting longer term may increase the likelihood of developing chronic pain.
The belief that pain and activity are harmful: In the initial injury responsible for the episode of low back pain is likely that some damage has occurred. However, this is installed quickly and the road recovery is to face the pain to some extent and continue their normal activities. A gradual return to exercise, not to be reversed if the pain increases to some As is the way to return to normal. If the patient believes the pain and physical activity to avoid harmful activities and not follow the road to recovery, the choice to relax in response to pain.
The adoption of a sick role: People vary greatly in how they respond to pain, disease or injury, to ignore and practice of completely stopping all activity. Choosing to rest as a therapy for pain is a bad choice and that choice of a sick role is a risk factor for the development of disability.
Low mood and social withdrawal: The stress of a pain problem may induce low mood or depression. This can have a significant effect in patients thought, beliefs, attitudes and actions which tend to choose the rest, and poor coping strategies. The patient may also withdraw from normal social contacts, avoid going out and in some cases suffering from panic attacks and agoraphobia become.
Practice Treatment does not fit better: The practitioner can do much to change the status of the patient, both for good and for ill. Avoid the establishment of disability and must choose active therapies to improve the rather passive role of therapies applied to the patient by the therapist. Physiotherapists should not criticize other diagnoses, but try to clear up any confusion on this matter and the expectation of a technological solution should be minimized. Physiotherapists have a responsibility to therapies make the best evidence and encourage patients to remain in employment.
Lawsuits and Compensation: spinal pain conditions are often the result of injury or work-related problems. This can lead to legal claims for compensation, which can take years and involve repeated consultations with different specialists. Some people find it hard to move forward while the process is ongoing and become stuck in their situation, others fear being shot if they something active and withdraw from normal activities.
Other complications in a good recovery from back pain include having episodes of back pain before, taking a long time sick, have problems and low job satisfaction, working hours and unsocial and a heavy work or family support helpful.
